What is the correct way to setup the distance constraint?

All of the steps you took are correct, what’s missing is the next step of adusting the Parent Offset and Child Offset attributes on the Distance Constraint. These determine what relative position on the parent and child respectively to constrain together.
Per default, the center of the parent and child are constrained. But in this case, since we are talking about a 2-point muscle constraint, you’ll want to instead constrain the tip of the muscle to the surface of the parent, the head in this case.

Unfortunately even adjusting the attachment points to be exactly where I want them, the mesh doesn’t seem to actually “hang” to this distance “string”, it falls all the way to the ground
Here’s what you should expect.
However I noticed that if there is any Scene Scale other than 1.0, it doesn’t rotate like you would expect. Smells like a bug, so try without any scale and compensate by adjusting Gravity.

Try setting Scene Scale to 1.0 on the solver.
Oh, yeah! it seems to work now, is there some correct scene scale to the scene units (millimeter, centimeters, etc)?
Yes, with a scene scale or 1.0 it respects Maya’s scale, which is typically centimeters. So if your character is 180 units tall, then that’s a scene scale of 1.0, if your Maya preferences are set to centimeters.
